Carolyn Diane Richards, 60, of Forest City, passed away Wednesday, May 18, 2016.

A native of Polk County, she was the daughter of the late Clarence Ed and Elma Katherine Turner Richards.

She is survived by two sons, Sheldon Lewis and Chad Richards, sister, Kay Gee; brothers Phillip Richards and Wayne Richards.

Services will be announced at a later date.

Crowe's Mortuary is assisting the family.
